Privacy Policy

Client records are kept according to the The Health Insurance Portability & Accessibility Act (HIPAA).

A client records release form will have to signed and dated prior to the release of any records. This form will be valid for a year from the date signed; unless permission is revoked by the client.

Client will be given a written notice of Privacy Practices and policies that are implemented to protect patient information. A signature of their consent for agreement with the practices and date will be required for services.
